As part of the PRE-EMPT study, you are invited to complete the following questionnaires, which will take about 90 minutes in total (2 hours including the computer tasks). Your completion of these questionnaires is completely voluntary. There are no foreseeable risks associated with these questionnaires.
Your responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and de-identified. If you have questions at any time about the questionnaires, you may contact the Principal Investigator of this study, Dr Geoffrey Tan, at 6389 2000.
